Ukrainian Research Institute of Children and
Adolescents Health Care was founded in Kharkov in October 1922 as the first
Ukrainian State Research Institute of Motherhood and Childhood Protection
(Decree of Ukrainian People’s Commissariat for Health Protection No. 174 of
28.10.22) with S.M. Yampolsky as the first director. By order of the Ministry
of Health of Ukraine No. 40 of 18.01.65 the Institute war reorganized into
Kharkov Research Institute of Children and Adolescents Health Care. From that
time on the Institute has been a leading scientific and research institution in
Ukraine dealing with the problems of schoolchildren and adolescents health
protection. By order of the Ministry of Health No. 122 of 07.08.92 the
Institute was renamed Ukrainian Research Institute of Children and Adolescents
Health Care.
The principal scientific research objectives
are:

organization of medical assistance to
schoolchildren and adolescents;
-
studying the state of health in schoolchildren
and adolescents in various regions of Ukraine and the influence of
environmental and hereditary factors on its development;
-
working out and testing new methods for
prevention, diagnosis and treatment of various non-infectious diseases and
functional disorders in prepubertal and pubertal age;
-
development of schooling regimens for
schoolchildren, students of vocational schools and technical colleges and
formulating criteria for professional orientation.
Since 1996 the Institute has been providing scientific
supervision of national programmes: “Children of Ukraine”, “Family planning”,
“National programmes for patriotic upbringing, promoting healthy ways of life,
developing spiritual values and strengthening moral principles of society”, and
of the integrated programme “Diabetes mellitus”.
Since 1999, according to the agreement signed with
Canadian association for international health protection and Canadian agency of
international development, researchers of the Institute have been participating
in working out joint Ukrainian – Canadian project “Youth for health”.
For the past years the following well-known scientific
schools have been formed at the Institute: “Arterial hypertension and
atherosclerosis precursors in children and adolescents”, “Rheumatology of
children and adolescents”, “Pediatric and adolescent endocrinology”, “Hygiene
of children and adolescents”.

INSTITUTE CLINIC
The clinic of the Ukrainian Research Institute
of Children and Adolescents Health Care is one of its structural units
functioning as a treatment and prophylactic institution and being the
test-ground for research work and for qualification upgrading for physicians:
pediatricians, therapeutists, cardiorheumatologists, endocrinologists,
psychiatrists and gynecologists, as well as for paramedical personnel.
One of the general trends of the Institute activities
is treatment and consulting assistance to children and adolescents (aged 3-18)
from various regions of Ukraine. Structure of the Institute also includes a
consulting polyclinic, an in-patient department (220 beds), and a laboratory
complex.
At the consulting polyclinic (250 visits a day) the
patients are received by various specialists (23 professional specialization
spheres).
The in-patient department includes the following
sections:
1) section of internal diseases (60 beds) for children
and adolescents with cardiovascular and rheumatic diseases, with systemic
lesions of connective tissue and with digestive system pathology;
2) rehabilitation center (30 beds) for children and
adolescents who suffered from Chernobyl catastrophe consequences;
3) section of endocrinology (60 beds) for patients
with diabetes mellitus, thyroid gland diseases, growth and sexual development
delay, etc;
4) section of psychiatry (40 beds) for patients with
non-psychotic psychic disturbances;
5) section of
pediatric gynecology (30 beds) for teen-age girls with gynecological diseases.
For examination of the patients the Institute has a
well-equipped up-to-date laboratory-and-diagnostic base for conducting over 120
types of tests.
At the clinico-diagnostic laboratory in addition to
conventional methods of blood investigation the following special methods are
used: thrombocytometry, adhesion of platelets test, thrombocytogram, clot
retraction test, erythrocytometry with determination of MCD (mean corpuscular
diameter), MCV (mean corpuscular volume), MCA (mean corpuscular average),
corpuscular sphericity; determination of glucose-6-phoshate dehydrogenase,
blood osmotic fragility test, and reticulocytogram. The biochemical laboratory
carries out determination of glycosylated hemoglobin, oxyproline and average
molecules. The bacteriological laboratory carries out testing of gastric
bioptates for determining Campylobacter availability. At the radiological
laboratory functional studies using radiopharmacological drugs labelled
with 131I, and scanning using drugs labelled with 99TC are
performed. Hormones and bioactive substances are studied by radioimmunologic
methods with the use of commercial kits.
At the X-ray rooms (both at the in-patient clinic and
at consulting polyclinic) the following special methods are used: bone,
articular and thoracal roentgenoscopy and roentgenography using functional
tests and gastro-intestinal roentgenoscopy, and roentgenography employing
artificial hypotonia and lateroscopy; irrigoscopy, i.v. urography; bone,
pulmonary, renal and adrenal tomography, and mictional cystography.
High quality of the
services at the Institute is ensured by availability of such units as:
laboratory of clinical immunology, laboratory of age-related endocrinology,
laboratory of medical genetics, and laboratory of functional diagnosis.
For treatment of patients
specialists of the section of rehabilitation therapy are using up-to-date
physiotherapeutical methods which make it possible to decrease medicamental
loads on the patient’s organism: decimetric electrowave therapy,
electrophoresis of ganglioblockers and various curative substances,
electroaerosols, lazeromagnitotherapy, ultrasound and acupuncture. The clinic
has balneotherapeutic unit where patients are treated with pine, salty-pine,
pearl, carbon-dioxide, iodo-bromine and sage baths, underwater douche-massage,
etc.
The scientific personnel
of the clinic is constituted by highly qualified specialists: 2 Honoured
Scientists and Technologists of Ukraine, 5 Honoured Health Care Practitioners
of Ukraine, 12 M.Ds, and 42 Candidates of Sc. (Med.), 49 doctors and
researchers of the highest and the first qualificational classes.

The main objective of scientific activities of
the section is morbidity prophylaxis in children and adolescents who suffered
from Chernobyl catastrophe. It relates to those evacuated and moved from the
contaminated territories. Since 1986 such children have been under close
observation as regards their development depending upon duration of radiation
exposure. Special attention has been paid to the state of cardiovascular system
at the remote stages of post-Chernobyl situation. The role of radiational
stress has also been studied.
In recent years the researchers of the section
have concentrated their efforts on monitoring the health state of the children
born from fathers-eliminators of Chernobyl catastrophe consequences.
Pathogenetic aspects of their health formation during the period of
neurohormonal changes have been considered, and the role of immuno-endocrinal
interaction at the stages of sex maturation has been closely studied.
One of the trends in the work of the section is
solving the problem of provoked genetic apparatus in the cells of the
offsprings of fathers-eliminators of the catastrophe and determining
phenotypical peculiarities in such children.
Researchers of the section participate in
working on the problem of primary arterial hypertension by studying
pathogenetic mechanisms of early disorders in cerebral hemodynamics, and carry
out investigations studying connective tissue metabolism impairments in various
forms of its dysplasia.
A significant place in scientific activities of
the section belongs to studying children's psychosomatic health and their
adaptation abilities that prove the influence of emotional state on the level
of non-specific immunologic reactivity and general morbidity rates in children.
SECTION OF ADOLESCENT PATHOLOGY

Researchers of the section traditionally deal
with the problems of cardiovascular and rheumatic diseases. Various aspects of
diagnosing, treatment and rehabilitation of patients with rheumatism,
rheumatoid arthritis and other inflammatory articular lesions as well as
non-rheumatic cardiopathies and vascular impairments have been developed at the
section.
The researchers of the section were the first
in Ukraine (and outside Ukraine) to begin studying the problem of
atherosclerosis precursors in adolescents with determining the participation in
such processes of certain links of circulatory system, including the coronary
link, the system of heart functioning regulation (central and vegetative
nervous systems, and their mediators), endocrinal system, lipide metabolism,
hemostasis system in parallel with risk factors for the disease development
(arterial hypertension, heredity and hormonal dysbalance).
In recent years special attention has been
given to studying the problems of connective tissue systemic disorders of
congenital and acquired origin and genetically conditioned dysplasia
(arrhythmia, prolapse of the valves, systemic lupus erythematosus,
peryarteritis nodosa, dermatomiosites, etc), as well as to prevention of
complications caused by such diseases.
A significant place in scientific research
belongs to the problems of primary arterial hypertension in adolescents, to
mechanisms of organic disorders caused by the disease, and to predicting
complication in various vascular fields (central, coronary, and renal) and
metabolic impairments with metabolic syndrome formation.

Basic trends of scientific and practical
activities of the section are connected with the problems of neuroendocrinal
regulation of female gonadal system formation (in norm and pathology), and the
effect of various exogenic and endogenic factors (extragenital diseases,
increased physical loads, conditions of upbringing and certain factors in
professional training).
For the recent 10 years certain new methods
have been introduced into practical treatment of adolescent girls with
gynecological diseases (sexual development delay, juvenile uterine bleeding,
syndrome of polycystic ovaries, etc). The methods have been patented.

Scientific research at the section is connected
with the problems of diabetes mellitus (d.m.)
in children and adolescents, their growth and sexual development disorders,
obesity, functional disturbances, and with thyroid gland diseases. The results
of the research made it possible to reveal certain peculiarities in d.m. and
obesity pathogenesis, in growth and sexual maturation pathology, to work out
new approaches to their treatment and prevention, and to ensure rehabilitation
and social adjustment of endocrinal patients. In the research up-to-date
laboratory and diagnostic equipment for radioisotope, immunofermentative,
ultrasound and some other highly precisive methods of investigation has been
extensively used.
Methods of treatment of d.m. complications and
sexual development delay in boys are patented in Ukraine, and these
technologies are widely applied by practicing doctors not only in this country
but also abroad.

Research work at the section is defined by
expansion of psychic diseases among children and adolescents in Ukraine, by the
problems of neuroses and neurose-like impairments, behaviour deviations and is
connected with the development of methods of correcting psychic and social
adjustment of such patients. Attention is also paid to studying the state of
psychic health in children and adolescents, who suffered from Chernobyl
catastrophe.
The results obtained made it possible to introduce
into practice at medical institutions of Ukraine certain technologies of early
diagnosis and rehabilitation of children and adolescents, and to develop a
conception of medico-psychological assistance to such patients.
THE DEPARTMENT OF
SCHOOLCHILDREN AND
ADOLESCENTS HYGIENE

The researchers of the department have been most
seriously working at solving vital problems in the field of schoolchildren and
adolescents hygiene.
The principal trends of their research include
studying the importance of sanitary and hygienic factors and environmantal
factors in the development of health state in juveniles, the conditions and
regimens of their work at various educational institutions (schools, colleges,
vocational schools, etc), and health state monitoring and prevention of
non-contagious diseases in schoolchildren.
Another object of research carried out by the
department is the study of the factors of in-school conditions and the learning
process, as well as pedagogical innovations, and determining their role in the
development of schoolchildren’s health state, as well as building up a system
of automated monitoring, expert estimation and prognosing the health state of
schoolchildren.
Today the researchers of the department are working at
an inventory of professions for professional orientation and training of
disabled children and adolescents with various defects of development. Special
attention has been paid to the problems of healthy ways of life, to programmes
and methods of introducing valeological education into school practice, as well
as to the development of a system of measures aimed at optimising recreation
and health improvement process for children with different levels of health
state.

The principal lead in the laboratory research
activities is connected with studying neuroendocrinal mechanisms of sexual
maturation during its physiological or pathological course, and the influence
of harmful exogenous and endogenous
factors upon the function of the hypothalamus-hypophysis-gonads-adrenals system
during prepubertal period and in puberty. Since 1986 the laboratory has been
monitoring the state of endocrinal system in children and adolescents who fell
victims to the consequences of Chernobyl catastrophe.
Special attention has been paid to determining the
role of biogenic monoamines, including indolamines of epiphysis, and the
character of their metabolism in formation of sexual system pathologies of
various origin, and to working out new approaches as regards its correction.
To determine the pathogenetic mechanisms of starting
and development of remote complications resulting from insulin-dependent
diabetes mellitus and disturbances of sexual development in children and
adolescents there has been carried out the study of the processes of free-radical
lipid peroxidation and the antioxidant system.
The laboratory researchers were the first in Ukraine
to begin studying the activity of ensymes,. of collagen and elastin metabolism
at various forms of connective tissue dysplasia in children and adolescents.
The laboratory employs biochemical and
immunofermentative methods that make it possible to estimate the levels of
tropic hormones of hypophysis, sex steroids and glucocorticoids, metabolism of
biogenic monoamines, vitamin content, the processes of lipid peroxidation, the
state of antioxidant system, and collagen and elastin metabolism.

The
principal trend of research carried out at
the laboratory is studying the peculiarities of immune system
functionality in children and adolescents at various stages of their organisms
development and maturation, as well as studying the mechanisms of the
development of autoimmune pathology and finding the ways of its prevention and
treatment.
Research work at the laboratory is aimed at revealing states of
immunodeficiency and immunopathological reactions in the course of rheumatic
diseases, connective tissue displasias, d.m., sexual development pathology in
children and adolescents, and at working out recommendations and individually
adjusted schemes of treatment employing up-to-date immunotherapy methods.
For the last few years special attention has been given to studying the
state of immunity system and to correcting its functioning in children and
adolescents who fell victims to the consequences of Chernobyl catastrophe, and
the children born from
parents-eliminators.
Researchers of the laboratory employ current immunological methods of
investigation, including determination of the state of cellular and humoral
link of immunity (T-and B-lymphocytes content and their functional activity
level); studying the factors of non-specific resistance of organism and
functional activity of phagocytes (complement content, lysozym content,
phagocytic number, phagocytic index, etc); investigating activity of cells in
the product of regulatory – inflammatory cytokines (necrosis factor of the
tumour, α, β and γ interferons, 1, 2, 4, 5, 6 interleukines,
etc) and presence of organospecific and tissue-specific auto-antibodies to
collagen, elastin, DNA, thyroglobulin and microsomal antigen. All that makes it
possible both to diagnose the diseases present and to predict the development
of other ones.

Laboratory
of medical genetics was opened in Ukrainian Research Institute of Children and
Adolescents Health Care in 1976. For many years now at the laboratory there has
been carried out research work as regards estimation of the role of hereditary
factors in development of rheumatic, endocrinal and gynecological diseases in
children and adolescents.
Today
the researchers of the laboratory are working on original ways and methods of
early diagnosing and preventing multifactor pathologies in children and
adolescents, especially those related to gynecology, endocrinology, and
cardiorheumatology, and are also determining the consequences of Chernobyl
catastrophe in children born from parents-eliminators and those evacuated from
the contaminated territories. High levels of qualification enable the
researchers of the laboratory to carry out genealogic, cytogenetic
(quantitative and qualitative chromosome analysis using differential colouring
method), immunogenetic (hystocompatibility system antigens), and dermatoglyphic
study.

The principal lines of research at the laboratory are
directed at studying the functional state of cardiovascular system in children
and adolescents with cardiac disturbances of various origin in those with
diabetes mellitus, in children and adolescents that fell victims to the
consequences of Chernobyl catastrophe, and
in those born from parents-eliminators. The results of the research help
to improve the reliability of diagnosing cardiovascular pathology and make it
possible to control the effectiveness of rehabilitation. Special attention has
been paid to investigating the mechanisms of resistance to emotional stresses,
and to working out computer–aided methods of quantitative estimation of the
state of nervous, cardiovascular and vegetative systems in children and
adolescents.
At the laboratory there have been developed
original diagnosing systems: computer–aided echoencephalography which makes it
possible to carry out systemic analysis of neurodynamic systems of the brain on
the basis of multidimensional spectroscopic analysis in echoencephalography;
computer-aided vegetology which aids in carrying out the estimation of the
stress on regulatory systems and the state of various links of circulation
regulatory system, as well as the capacity of hypothalamus integrative function
and of the nuclei of medulla, based on mathematical analysis of cardiac rhythm.
Alongside with that, the researchers of the
laboratory employ all the advanced technologies of functional investigation:
dopplerography, echocardiography, echosonography, thermovision, computer-aided
spirography, and other ways and methods which aid in timely diagnosis.
On the basis of the laboratory there functions the
Chair of medical informatics and information technologies in health care
management (Kharkov Medical Academy for Postgraduate Advanced Training).
SECTION OF SCIENTIFIC
ORGANIZATION OF MEDICAL ASSISTANCE TO SCHOOLCHILDREN AND ADOLESCENTS

Research work at the section is aimed at analyzing
health state of schoolchildren and adolescents from various regions of Ukraine
and at working out recommendations concerning the problems of medical
assistance to the growing generation at polyclinical and clinical institutions,
and also at developing regulations defining the functional duties of medical
and pedagogical personnel.
For a number of years the experts of the section have
been studying the problem of medical health services for male juveniles before
their army drafting.
The results obtained made it possible to work out
specialized instructions concerning set-up of independent polyclinics for
adolescents and methodological recommendations concerning day hospitals for
such patients. The project “Concepts of medical health services rendered at
present to juveniles due to be drafted” has been prepared and submitted.
Using to the data of statistical reports from regional
therapeutists researchers of the section together with the experts of the
Ministry of health of Ukraine annually analyze adolescent morbidity and prepare
a collected book of findings concerning the incidence of diseases among them.
The researchers provide organizational assistance in
the field of advanced training of specialists who are responsible for medical
assistance to such patients by giving their counsel on the current problems of
children and adolescent physiology and pathology during special information
courses on the basis of the Institute and on lecturing tours all over Ukraine.

Scientific
activity at the department is aimed at studying the information streams related
to the problem "Health protection of schoolchildren and adolescents",
carrying out patent and informational expertise of R a D and theses at all the stages,
determining the informational needs of the researchers and introducing
achievements of medical science into practice.
The problem of
information processing at the department is carried out by the experts highly
qualified in various forms of developing the system of scientific medical
information.
The work of the unit of
patenting and licensing is aimed at organization of studying the status and
tendencies of scientific development in the field of pediatric and adolescent
medicine for determining priority trends in scientific activities of the
Institute. The information database includes complecte specifications of
Ukrainian and Russian patents (authorship certificates) and patent information
sources related to scientific research problems dealt with at the Institute.
The translator of the
department possesses considerable experience in translating and abstracting
literature in foreign languages on medicine and related subjects.
Scientific medical
library contains home and foreign periodic publications, both original and
reference, reference information bank (RIB) which consists of a classified
collection of documents dealing with pediatrics, and school and adolescent
medicine (total list exceeds 52000 titles), all the publications issued by the
Institute and other information institutions, abstract journals, express
information, scientific reviews, books, monographs, etc.
